Diabetes Mellitus Screening at Less Than 24 Weeks of Gestation
Due to the lack of consistent evidence to indicate neonatal and maternal benefit from early diagnosis and treatment of gestational diabetes mellitus (GDM), the American College of Obstetricians and Gynecologists (ACOG) does not recommend routine screening for GDM before 24 weeks of gestation. ACOG continues to recommend screening for pregestational diabetes mellitus before 24 weeks of gestation, preferably at the onset of prenatal care, in patients with risk factors (Table 1). This guidance is prompted by the escalating prevalence of obesity and type 2 diabetes in reproductive-aged people and challenges with access to glucose screening, which have led to a higher incidence of pregnant individuals with undiagnosed type 2 diabetes (1).
